Official text

(1) Any person aggrieved by an order made by the officer under these rules may prefer an appeal before the Labour commissioner, Uttar Pradesh. The Appeal shall be made within sixty days of receipt of copy of the order of the officer.
(2) The appeal shall be accompanied with (form-57) -(a) the order appealed against;
(b) a non-refundable fee equivalent to five percent, of the amount of penalty levied on under such appeal;
(c) a statement of issues in dispute;
(d) documentary evidence relied upon;
(e) relief sought.
