Official text

Where any worker in an establishment contracts any disease specified in the Third Schedule of the Code, the manager or employer of the establishment shall send a notice electronically or otherwise in Form-12 to the concerned Inspector-cum-Facilitator, Chief Inspector-cum-Facilitator and Chief Medical Officer of the district within forty-eight hours from the date on which it comes to the knowledge of the employer or manager of the establishment.
