Rule 9. Notice of disease

1) Where any worker in an establishment contracts any disease specified in the third Schedule of the Code, the employer of the factory shall send a notice electronically or otherwise in Form-IX to Inspector-cum-Facilitator, Chief Inspector-cum-Facilitator and Chief Medical Officer of the district within seven days from the date on which it comes to the knowledge of the employer or employer of the establishment.

2) If any qualified medical practitioner attends on a person who is or has been employed in a establishment and who is or is believed by the qualified medical practitioner, to be suffering from any disease specified in the Third Schedule of the Code, he shall forthwith send a report electronically or otherwise to Chief Inspector-cum-Facilitator or District Labour and Employment Officer stating: -a) the name and full postal address of the patient,

b) the disease from which he believes the patient to be suffering, and

c) the name and address of the factory in which the patient is or was last employed.
