Official text

(1) The notice to be sent by the Competent Authority under sub-section (1) of Section 88 of the Code shall be in Form-XXI and shall be accompanied by a blank copy of Form-XXII.
(2) The statement to be submitted by an employer under section 88 shall be in Form-XXII.
(3) The report in respect of an accident under sub-section (1) of Section 88 may be sent to the Authority to whom the notice of such accident is required to be given.
